Wondering about fees and payment details? We’ve made it easy by outlining our consultation, surgery, cancellation, and payment policies below to help you understand all the financial aspects of your care at Baumholtz Plastic Surgery.
Consultation and Appointments
- Consultations with Dr. Baumholtz are $200 (in-person or virtual). This fee is non-refundable but may be applied to surgery if booked within 2 weeks of your visit.
- Surgery estimates are valid for 30 days.
Cancellations & No-Show Visits
If you need to cancel or reschedule, please notify us at least 48 hours in advance.
- No-Show Surgical Consultation - $200
- No-Show Other Appointments - $50
- A 25% non-refundable deposit is required to reserve your surgical date.
- Full payment is due 3 weeks prior to surgery, typically at your pre-operative appointment.
- We accept cash, credit/debit cards, and bank cashier's checks.
- Financing through 3rd-party providers is available. Your agreement is directly with the finance company.
If you cancel or postpone your procedure, your surgical deposit remains on your account as credit for surgery for up to 12 months from the date of payment. After that:
- If no surgery is scheduled and no communication is received, the deposit is forfeited.
- Deposits are non-transferable and cannot be used by another person or for another service.
- Extensions may be granted for medical necessity or documented hardship, but must be requested in writing before the 12-month expiration. Supporting documentation required.
- Patients are responsible for remaining in contact with our office to retain eligibility for deposit use.
- Patients who defer surgery and actively enrolled with our weight loss program will receive an extension for their surgical deposit.
- More than 3 weeks prior to surgery or for verified medical reasons: $500 fee
- Less than 3 weeks or no-show for pre-op: Deposit forfeited; new deposit required
- Under 14 days: Deposit forfeited + $2500 rescheduling fee
- Under 7 days, no-show, or failure to disclose medical info: 100% of fees forfeited
- Once a product is sold or a service is rendered, no refunds will be given.
- No guarantee is offered on surgical or non-surgical outcomes.
- Additional treatments or revisions may incur separate fees.
- Promotions must be used within 6 months.
- Discounts and packages are non-refundable and non-transferable.
- If surgery is rescheduled, pricing may change.
- Medical testing (labs, EKG, clearance) may be required. Non-compliance may result in cancellation without refund.
- Nicotine use must be discontinued 6 weeks before and after surgery. Failure to comply voids your surgical agreement without refund.
- Medical clearance, labs, pathology, and anesthesia fees are included unless otherwise stated.
- Facility-related fees due to rescheduling or cancellation are the patient's responsibility.
- If tissue samples are sent for testing, patients are responsible for associated costs not covered by insurance.
- A valid card must be kept on file for your account.
- It will only be charged with your prior approval for agreed-upon services.

You acknowledge that you have read, understand and agree to these terms. You affirm that you enter into this agreement voluntarily.
